SUBCONTRACTORS
Insurance and Business Requirements
In order for us to enter into a Contract Agreement for services with you, the subcontractor, the following information must be provided to us.
Certificates of Insurance evidencing your Coverage for the following must be submitted to us and verified as accurate and still in effect prior to you starting work on each project you are being considered.
Insurance Requirements:
1. Certificate of General Liability
General Liability: Minimum Coverage
- $2,000,000 General Aggregate
- $2,000,000 Products Comp/Op Agg
- $1,000,000 Personal & Adv. Injury
- $1,000,000 Each Occurrence
- $100,000 Fire Damage
- $5,000 Medical Expense
2. Certificate of Auto Liability
Auto Liability: Minimum Coverage
- $ 1,000,000 Combined Single Limit
3. Certificate of Workers Compensation Coverage A Statutory and Coverage B Employer Liability
Workers Comp: Minimum Coverage
- $ 1,000,000 Combined Single Limit
Required Endorsements:
The following language needs to be inserted in the Insurance Certificates in the appropriate spaces as set forth below:
- In Description of Operations section of certificate, insert the following required wording:
“Project (insert specific project name/number). Certificate Holder is included as an additional insured with respect to General Liability subject to policy conditions. See attached per project aggregate endorsement, and additional insured endorsement. *10 days notice of cancellation for non-payment of premium."
- In Certificate Holder area, insert:
Roofing & Restoration Construction Group, Inc.
(Tre: Note: BUSINESS ADDRESS TO BE PROVIDED)
- Additional Insured and Per Project General Aggregate Endorsement should be issued on a separate form referencing company and policy number. NOTE: Each time you issue a renewal or job-specific certificate, new endorsements must be provided. If the per project aggregate endorsement is not available, we require at least a $2,000,000 umbrella or excess liability policy.

1. W-9
2. Current Copy of State Contractor's License
3. Copy of Safety Manual
4. Business Licenses
5. Letter of Good Standing from the Secretary of State
